Meal Application


New!  Online meal application is now available for FREE and REDUCED-PRICED meals.  Select link below to apply for a 2017-2018 school year application.  Instructions on how to complete the online application are listed below.  Only one type of application is necessary to submit per household each school year (online or hard copy).

Roselle Road, Palatine, IL 60067-7336.  Follow directions carefully. Applications must be completed thoroughly or they will be returned or denied.

Additionally, if your household receives a letter stating that your child has been automatically approved for meal benefits, you do NOT need to complete a meal application.  Please ensure that all of your children are listed on the letter, and notify our department immediately if they are not. To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form.
